LOCATION: Embark at Ashburn 20116 Ashbrook Pl Suite 110 120, Ashburn, VA 20147

SALARY: $55,000 - $65,000 annually

 

As an Embark Case Manager you will be supported by an experienced Clinical Director and Administrative Team that are all focused on the mission to facilitate lasting positive change for young people and their families at the IOP and PHP levels of care. Come grow with us!

 

Case managers focus on coordinating and advocating for services and work closely with therapists and mental health professionals and ensure clients receive the appropriate therapeutic interventions. Collaborate with the treatment team to develop treatment plans, monitor progress, and make referrals to therapy services when needed. Play a crucial role in facilitating access to therapy services by coordinating appointments, assisting with insurance, financial matters, and helping clients.

 

Embark operates facilities across the US that support adolescents, young adults and their family’s mental health and well-being by offering a full continuum of care from Long Term Residential and Wilderness Therapy environments to our IOP/PHP clinics to virtual outpatient care. Our local IOP & PHP clinics are a critical part of our strategy to increase access to life-saving mental health treatments for families with insurance.

Responsibilities

Essential Duties and Responsibilities:

  1. Complete education assessment upon intake
  2. Create Education Success Plan for client
  3. Schedule weekly or as needed check ins with client
  4. Create line of communication with school contact to accommodate any academic needs of client
  5. Track client attendance, including any return to school documentation
  6. Complete case consults with referring clinician upon admission and discharge
  7. Facilitate IOP and PHP sessions and scheduling
  8. Clinical support, including attendance at clinical staff meeting weekly, conflict resolution with students, and individual case management meetings with students, when needed.
  9. Medication management coordination and support to ensure the consistent and accurate delivery of prescribed medications to students.
  10. Assesses clients for suicide risk and performs appropriate screening for suicide risk when needed.
  11. Provides care and monitors symptoms for individuals with moderate or high risk of suicide.
  12. Collaborate with clinical team and parents to develop and implement treatment schedule
  13. Client and family discharge planning.
  14. Facilitate pre-treatment with Enrollment Director as needed
  15. Maintain client treatment schedules
  16. Facilitate student access to health services (i.e., medical, dental, vision, etc.) through a process of fielding the initial complaint, doing assessment, and communication with the Clinical staff to schedule the appointments.
  17. Case management phone contact with parents of students to update regarding student health issues, household progress, settling in of new students, and other general progress.
  18. Timely documentation in reports and Best Notes.
  19. Work with students to teach and supervise household responsibilities and life skills.
  20. Attend treatment team meetings.

Qualifications

Required Knowledge, Skills, and Abilities:

1. Experience working in education and behavioral health a preference.

2. Proficiency in assisting the administration of suicide screens such as C-SSRS.

3. Effective communication skills.

4. Ability to organize, prioritize and manage schedules and deadlines.

5. Ability to remain in control during stressful situations and facilitate conflict resolution.

6. Ability to be both firm and friendly and demonstrate appropriate boundaries with students.

7. Maintain a professional demeanor at all times.

8. Attention to detail.

9. Team player.

10. Flexibility a must.

11. First Aid & CPR certified.

 

Education and Experience:

B.A. in Social Services field or combination of education and experience equal to a minimum of four years relative experience and training in youth services or mental health treatment. Master’s Degree in related field of study strongly preferred.
