Salary: $45,000 - $55,000 a year

Location: 7 Regent St, Suite 701, Livingston, NJ 07039(On-Site)

Schedule: Monday - Friday 

 

Embark Behavioral Health is looking for a Clinic Case Manager who aligns with our mission of reducing adolescent and young adult anxiety, depression and suicide to staff our OP, IOP and PHP clinic in Livingston, NJ.Come join our team of empathetic professionals who are passionate about creating joy and healing generations together!

 

As a Case Manager you will be supported by an experienced Executive Director, Clinical Director, Therapists and Administrative Team that are all focused on the mission to facilitate lasting positive change for young people and their families.

Responsibilities

  • Complete education assessment upon intake
  • Create Education Success Plan for client
  • Schedule weekly or as needed check ins with client
  • Create line of communication with school contact to accommodate any academic needs of client
  • Track client attendance, including any return to school documentation
  • Complete case consult with referring clinician upon admission and discharge
  • Facilitate PHP sessions and scheduling
  • Clinical support, including attendance at clinical staff meeting weekly, conflict resolution with students, and individual case management meetings with students, when needed.
  • Medication management coordination and support to ensure the consistent and accurate delivery of prescribed medications to students.
  • Collaborate with clinical team and parents to develop and implement treatment schedule
  • Client and family discharge planning.
  • Facilitate pre-treatment with Enrollment Director as needed
  • Maintain client treatment schedules
  • Provide student access to health services (i.e., medical, dental, vision, etc.) through a process of fielding the initial complaint, doing assessment, and communication with the Business Office staff to schedule the appointments.
  • Case management phone contact with parents of students to update regarding student health issues, household progress, settling in of new students, and other general progress.
  • Timely documentation in reports and Best Notes.
  • Work with students to teach and supervise household responsibilities and life skills.
  • Attend treatment team meetings.

Qualifications

  • Minimum Bachelor's Degree in Behavioral Sciences or Education
  • Experience working in education and behavioral health a preference
  • A desire to facilitate client growth through a team approach, working with other care providers.
  • Ability to plan, implement, and adjust a course of treatment as required.
  • Ability to pass a state background check
  • Ability to maintain a calm demeanor when speaking with families in crisis.
  • Knowledge of DSM-V disorders and other pertinent issues facing students and their families.
  • Effective communication skills.
  • Ability to organize, prioritize and manage schedules and deadlines.
  • Ability to remain in control during stressful situations and facilitate conflict resolution.
  • Ability to be both firm and friendly and demonstrate appropriate boundaries with students.
  • Maintain a professional demeanor at all times.
  • Attention to detail.
  • Team player.
  • Flexibility a must.
  • First Aid & CPR certified.
