Mental Health Case Manager – Empower Teens & Families | Atlanta, GA

Pay: $24 - $26 per hour, DOE

Schedule: Full-Time, Monday - Friday (flexible)

Location: 960 North Point Pkwy Suite 450, Alpharetta, GA 30005

As a Case Manager, you’ll play a vital role in delivering client-centered, trauma-informed care. You’ll coordinate essential mental health services, collaborate with an integrated care team, and ensure clients receive evidence-based interventions that foster growth and healing.

Responsibilities

  • Educational & Executive Support: Empower students by conducting education assessments, creating personalized success plans, and leading dynamic daily executive functioning activities that spark academic and cognitive growth.
  • Client Monitoring & Safety: Be a trusted guide by scheduling check-ins, tracking attendance, managing return-to-school documentation, and performing daily emotional wellness check-ins to ensure client safety and well-being.
  • Clinical Coordination & Facilitation: Play a key role in the treatment process—collaborate on individualized plans, facilitate IOP/PHP sessions, manage schedules, and support smooth admissions and discharges.
  • Family & Team Collaboration: Build strong connections by working closely with parents, clinical teams, and schools to share progress updates, resolve challenges, and create effective discharge plans.
  • Administrative & Documentation: Keep everything running smoothly by maintaining accurate reports, participating in treatment team meetings, and teaching essential life skills that help clients thrive.

Qualifications

  • B.A. in Social Services or equivalent experience (Master’s preferred).
  • Experience in behavioral health or education strongly preferred.
  • Familiarity with suicide risk assessments such as C-SSRS.
  • Strong organizational skills and ability to manage schedules and deadlines.
  • First Aid & CPR certification required. (Don't have that already or it's expired? We'll help you get it!)
