Pay Range: $26.00 - $29.00 per hour, DOE

Location: 370 N Wiget Ln #100, Walnut Creek, CA 94598

Schedule: Monday to Friday, Alternating Shifts

• Three to Four Days Per Week: 8:30 AM to 4:30 PM
• One to Two Days Per Week: 11:00 AM to 7:00 PM

Empower Youth on Their Healing Journey — Case Manager Needed at Embark

Embark Behavioral Health is seeking a compassionate, organized Case Manager to support our IOP and PHP programs at our beautiful clinic in Walnut Creek.

In this role, you’ll guide families through intake, coordinate education and career success plans, and facilitate IOP and PHP sessions. You’ll maintain communication with families, schools, and clinicians while supporting medication management, tracking attendance and progress, and helping plan thoughtful discharges.

• Conduct education assessments upon intake and develop individualized Education Success Plans tailored to each student’s needs.
• Coordinate and facilitate weekly or as-needed check-ins with students, while maintaining communication with school contacts to support academic accommodations and track attendance.
• Attend staff meetings, and assisting with student conflict resolution and case management as needed.
• Collaborate with the clinical team, parents, and the Enrollment Director to create treatment schedules, manage pre-treatment processes, and plan for discharge.
• Oversee medication management coordination to ensure accurate and consistent delivery of prescribed medications.
• Provide access to essential health services by assessing student needs and working with administrative staff to schedule medical, dental, and vision appointments.
• Provide career training and support to clients in our young adult program.
• Co-facilitate IOP and PHP groups.
• Attend multi-disciplinary treatment team meetings and track notes.
• Maintain timely and accurate documentation in Best Notes, conduct parent outreach for student updates, and support students in learning life skills and household responsibilities.

• Education & Experience: Bachelor’s degree in Behavioral Sciences, Education, or a related field required; experience in education and behavioral health strongly preferred.
• Working knowledge of DSM-V disorders, ability to develop and adjust treatment plans, and experience supporting students and families in crisis with professionalism and empathy.
• Strong verbal and written communication skills, ability to set and maintain healthy boundaries, and a calm, supportive approach to conflict resolution.
• Proven ability to manage schedules, prioritize tasks, meet deadlines, and adapt in a fast-paced, evolving clinical environment.
• Must be CPR and First Aid certified and able to pass a state background check.
