Embark Behavioral Health is looking for a Continuing Care Specialist who plays a critical role in guiding individuals and families through Embark’s full continuum of care. This role supports clients as they transition between levels of treatment, helping them identify the right next step based on evolving clinical, emotional, and practical needs. By providing resources, advocacy, and consistent guidance, this individual helps clients navigate care options ranging from virtual counseling to residential services—supporting continuity, stability, and long‑term well‑being throughout their journey.

Responsibilities

  • Guides clients and families in recognizing the value of continuing care within Embark programs, recommending the most appropriate care pathway based on clinical, financial, and logistical needs.
  • Builds trusted relationships with clients, families, and referral sources by demonstrating strong knowledge of Embark services, addressing concerns, and supporting informed decisions around continued care.
  • Manages an active pipeline of continuing care opportunities, independently nurturing relationships and coordinating next steps to support client movement across programs.
  • Collaborates with Embark programs nationwide to develop and execute individualized continuing care plans, ensuring alignment across step‑down, step‑up, and lateral transitions.
  • Coordinates a seamless continuation of care process by securing required documentation, completing insurance verification, and preparing clients and families for program enrollment.
  • Communicates consistently with internal teams, external partners, and referral sources to support a positive client experience from discharge through readmission or transfer.
  • Maintains accurate, timely documentation of client status, decisions, and care coordination activities to ensure continuity, compliance, and data integrity.
  • Identifies and escalates barriers to continuation of care, applies best practices, and remains receptive to feedback and coaching to continuously enhance the client and family experience.

Qualifications

  • At least one year of experience in the behavioral health field in an intake, case management, outreach, admissions, or care coordination capacity.
  • At least one year of experience in a consultative, sales‑oriented, or conversion‑focused role supporting client or family decision‑making.
  • Demonstrated ability to build trusted relationships with clients, families, program leaders, and referral partners across care settings.
  • Experience collaborating with multidisciplinary stakeholders, including clinicians, case managers, educational consultants, and mental health professionals.
  • Strong attention to detail with the ability to accurately document client needs, decisions, and care coordination activities.
  • Comfort working in a remote environment using video conferencing, telehealth platforms, and CRM or EMR systems (Salesforce experience preferred).
  • Interest in or experience navigating insurance processes and discussing coverage and financial considerations with families (preferred).
  • Bachelor’s degree in mental health or a related field and experience working with adolescents, young adults, and families in outreach, admissions, or marketing capacities (preferred), with the ability to pass a state background check and travel up to 15% annually.
